How to Enroll a Patient

There are two ways to start the Enrolling Members process:

  1. Click  in the upper right corner
  2. Click Members, then

From there, simply fill the fields with the appropriate information. A few notes:

  1. (M) = Monthly /  (Y) = Yearly
  2. In the event of a monthly payment, you will see an amount due today, and total contract amount, since the two differ.
  3. The email address entered on the final page will serve as the username for that member to access their plan details online through vision-hq.com.
  4. You will be provided with a confirmation page at the end of enrollment. Your patient is emailed a receipt of the transaction, providing confirmation of enrollment.
